- Accomplished Admission Forms
- Certificate of Honors (If Applicable)
- Eligibility to Transfer/ PEPT Result/ Balik-Aral
- Additional for GRADE 1:
- Accomplished ECD Checklist
- Kindergarten Certificate of Completion
- Additional for JUNIOR HIGH SCHOOL
- For GRADE 7: Parent’s ITR or Certificate of Non-Taxable Income
- For GRADES 8-10: ESC Grantee Certification
- Additional for SENIOR HIGH SCHOOL
- For GRADE 11: SHS-VP Recipient Certificate, Grade 10 Certificate of Completion
